Welcome student Athletes. We encourage all athletes to have their sports physical completed by their primary care provider.

1. 运动身体

If you plan to have your 运动身体 done at the Student Health Center, you must bring your completed medical history forms provided by Athletic Training with you to your appointment. Prior to scheduling an appointment for an athletic physical, ensure you have received an acceptance email from athletics and are able to access the software program and print off the completed medical history forms. If you fail to bring these completed forms with you, your physical exam may be rescheduled.

  • Prior to scheduling an appointment for an athletic physical, ensure you have received an acceptance email from athletics and are able to access the software program.
  • Complete all your forms on the program.  Be sure to “save” each form when completed.
  • After electronically signing it, print and bring the forms to your scheduled appointment.
  • If any problems with the software program access or log-in, please contact your coach or athletic trainer right away.


2. Sickle-Cell Trait Blood Test

All student athletes must have proof of a sickle-cell trait test. If you are unable to locate your results, you can have your primary care provider perform the test or you can schedule this test at Student Health Center.

All students born in the United States after 1990 had this test done as part of their newborn blood screening.  Some of these tests may be referred to as “Newborn profile”, “Hemoglobin electrophoresis”, “Hemoglobin S solubility”  as well as “Sickle-Cell screening”.   To avoid repeating this test, you should attempt to locate the results.

Some suggestions for locating the results:

  • Medical Records at the hospital in which you were born
  • 儿科医生
  • Primary Care Provider
  • 父母 or guardians

If you are UNABLE to locate your results, you will need to have this test repeated. It is a blood test, which can be done at your primary care provider’s office, or at Student Health Center by appointment.

If you choose to have this completed at Student Health Center:

  • Bring your insurance card to the appointment or a copy of the card (front and back). 

  • Results can take up to 1 week, therefore please ensure you have this completed prior to the start date of your sport. Failure to complete the sickle-cell testing may delay your participation in the sport.
